Transitional Care Program

In fulfilling our Mission of having a place where community members can find opportunities to improve their health and well-being, the Transitional Care Program was created.

This unique group of programs is for you if you are transitioning from, or managing a medical condition. All programs need a medical referral and our professional staff will work with your health care professional so you can achieve a healthy lifestyle for years to come.

The different concentrations of programs are medically based and geared towards you, the participant.

Arthritis Fitness (Aqua)

You are eligible to participate if you suffer from or have been diagnosed with arthritis, autoimmune disease or musculoskeletal conditions with joint mobility impairment.

Program:

  • One month in length
  • Pre/post assessment
  • Two Arthritis Foundation Aqua classes per week
  • Educational lecture/workshop
  • Progress reports to referring source
  • Limited access to the facility

Cancer Fitness

You are eligible to participate if you are in remission from cancer or are ready to transition from a supervised program into an independent fitness program.

Program:

  • Eight weeks in length
  • Individualized
  • Pre/post assessment
  • Five 30-minute personal training sessions
  • A one hour dietitian appointment
  • Educational lecture/workshop
  • Progress reports to referring source
  • Full access to the facility

Cardiac Fitness

You are eligible to participate if you are in post-cardiac rehabilitation looking to improve your cardiac function and transition into general fitness.

Program:

  • Eight weeks in length
  • Pre/post assessment
  • One equipment orientation
  • 24 group cardio workouts
  • Four hour long sessions with a registered dietitian
  • Educational lecture/workshop
  • Progress reports to referring source
  • Limited access to the facility

Diabetes Fitness

This program will help educate you about proper exercise and nutrition which can help manage the disease and its symptoms. Completion of a Diabetes Nutritional Education class lead by a registered dietitian is a prerequisite.

You are eligible to participate if you have diabetes or maybe experiencing pre-diabetic symptoms.

Program:

  • Six weeks in length
  • Pre/post assessment
  • Three hour-long sessions with a registered dietitian
  • Individualized exercise program (updated twice)
  • Educational lecture/workshop
  • Progress reports to referring source
  • Full access to the facility

Orthopedic/Pre-Habilitation

You are eligible to participate if you are preparing to have orthopedic surgery or returning to exercise after orthopedic surgery or rehabilitation.

Program:

  • Three months in length
  • Pre/post assessment
  • Individualized exercise program (updated three times)
  • Two supervised group training sessions per week
  • Educational lecture/workshop
  • Progress reports to referring source
  • Limited access to the facility

Transitional Care (General)

You are eligible to participate if you have been discharged from medical care or a hospital stay and looking to transition into basic fitness with some supervision.

Program:

  • Three months in length
  • Pre/post assessment
  • Individualized exercise program (updated three times)
  • Educational lecture/workshop
  • Progress reports to referring source
  • Full access to the facility

Weight Management

This program strives to improve your overall health and well-being through a team approach to exercise, nutrition and lifestyle management.

You are eligible to participate if you need to improve your health due to weight related issues.

Program:

  • Three months in length
  • Pre/post assessment
  • Three hour-long sessions with a registered dietitian
  • Three hour-long sessions with a behavior modification specialist
  • Twenty four hour-long Personal training sessions
  • Glucose assessment
  • Educational lecture/workshop
  • Progress reports to referring source
  • Full access to the facility
